You can use the form to request yearly Annual In-Person exams, 30 min telemedicine meds checks and 60 min virtual or in person full rechecks. As well as targeted 30 min and full 60 min behavioral coaching appointments with your Behavior Therapy Technician (current clients only)

You can use the platform below for virtual appointment scheduling, just keep in mind its all in Mountain time zone. For in-person appointments, reach out directly or request times here.
